We offer the most modern surgical facilities in Arizona, dedicated to the LAP-BAND. The seminars are free, but registration is required as space is limited. Fill out the on-line form below to register for a LAP-BAND seminar or if you wish to make an appointment.
If you are coming for an appointment there are some forms you can fill out beforehand to facilitate your visit. For those forms click here. We will also send you a link to our on-line medical record so that you can fill out your medical records at home and they will be available to us when you come to the office.
For appointments we see patient's all day Monday , all day Thursday, and Friday mornings and for our out-of-town patients we have limited availability one Saturday a month (when we have our seminar). Please check our hours and fill out the form below for a schedule, or you may also call the office at 602- 476-2334 or 602-234- SLIM. Please note our office is closed on Friday afternoons-- and the best time to reach us is Tuesday or Wednesday.

We will bill your insurance company for the visit. If you are not covered, the cost of the consultation is $100. If you have insurance, please check with your company to see if you will be required to have a referral prior to our appointment. Please note that our practice is limited to patients between the ages of 14 and 60.
