We offer the most lap-band experience in Arizona, dedicated to the LAP-BAND and the Gastric plication. The seminars are free, but registration is required as space is limited. If you are coming for an appointment there are some forms you can fill out beforehand to facilitate your visit. We will also send you a link to our on-line medical record so that you can fill out your medical records at home and they will be available to us when you come to the office.

We will bill your insurance company for the visit. If you are not covered, the cost of the consultation is $100. If you have insurance, please check with your company to see if you will be required to have a referral prior to our appointment. Please note that our practice is limited to patients between the ages of 14 and 60.
