Our staff is dedicated to helping you on your journey - and we have a lot of people to help you. But first, help us to learn about you. This will help you go through considerations for the LAP-BAND. It takes some time to read and go through, so we suggest you download this form here and print it out. This will save you time, and you can use that time to ask us questions as you go through them.

Once ready, you can have your appointment scheduled, you can fill out your medical history on-line or at home. This will save you printing out the medical history form. We still want you to read through the consideration form above. We ask all our patients to read this and fill it out at their appointment. It is some simple considerations about weight loss surgery. By downloading it ahead of time, it will save you time when you come to our office. We expect you to have some questions when it comes to the information in these forms.

Consent Form Consent Form
This is a copy of the consent form we use for patients prior to surgery. It is a comprehensive list of various items that can occur during and after surgery. We do not ask you sign this form until after you have had the pre operative class for the band, but looking the form over before your appointment might generate some questions to ask the physician. We will go over risks and potential complications from surgery -- and you can listen to our podcast about them. In spite of everything we do, and everything you do to minimize the risks of surgery, still things will happen. Our job is to make you aware of the things you can do to prevent complications, be aware of what they are, and we want you to know we are here to help you every step of the way. We encourage you to ask questions, we will give you a lot of material including our Workbook for lapband patients to make your experience the best.
